• Passports will not be issued
• Forms available at Clinic or prior to Clinic at: 15 Princess Street (Monday – Friday – 9 a.m. – 4 p.m.)
• Completed forms and documents will be mailed at no charge – Original birth certificates must be mailed along with copies of photo I.D. (Drivers’ Licence, Health Card) which must be signed by your guarantor
• Photographer on site, as well as document copying

• Child (3-15 Years) $37.00
