![]() Member of Parliament for Chatham-Kent Essex Dave Van Kesteren is sponsoring two Passport Clinics during the summer. The first will take place at the Knights of Columbus Hall at 20 Dupuis Street in Tilbury on Monday, July 19. On Wednesday, August 18 a second clinic will take place at Glad Tidings Community Church in Blenheim. Details of the August clinic will follow in the weeks to come. In response to numerous requests and recommendations, THE TILBURY CLINIC WILL RUN BETWEEN THE HOURS OF 3-7PM. This allows constituents who have previously been unable to attend during the traditional hours of 10am-2pm to take advantage of the service of helping people to complete application forms, have photos taken and obtain copies of necessary documents. PLEASE NOTE THAT PASSPORTS ARE NOT ISSUED AT ANY OF THESE CLINICS. Completed applications will be mailed to Passport Canada from the constituency office in Chatham at no charge on Friday, July 23. This allows applicants several days to complete outstanding items. Completed applications can be dropped at either the Chatham or Leamington office or at the Knights hall in Tilbury no later than 4pm, Thursday, July 22 to catch the free mailing. There will be staff on hand to distribute application forms. The charge for an adult (16 years old and above) application is $87.00. The costs for children are $22.00 (under 3 years old) or $37.00 (ages 3-15). Payment must be included with your application and can be made by certified cheque or money order payable to The Receiver General for Canada; or by major credit card (section on application to complete). A photographer will be available to take required photos at a cost including tax of $18.07. Payment for the photos can be made in cash or by major credit card. If you pick up forms to complete in advance make sure that you follow the enclosed instructions.
